Wiggly Explorers' Acknowledgement Agreement

I, the parent / legal guardian, understand that participation in the sensory classes with Wiggly Explorers involves activities that may carry inherent risks of injuries (including but not limited to: bumps, bruises, falls, etc. during toddler play and interaction).

 

By signing up for class, I acknowledge and agree to the following:

 

Parental Supervision is Mandatory: I understand and agree that I must remain present and within close proximity to my child(ren) at all times before, during, and after the sensory play class. I am solely responsible for supervising, monitoring and ensuring the safety and behavior of my child(ren) while participating in any activities organized by Wiggly Explorers.

 

Assumption of Risk: I voluntarily assume all risks associated with participation in all group activities, including but not limited to risks of physical injury, emotional distress, choking and/or accidents that may occur doing sensory play classes.

 

Waiver of Liability: I hereby release, waive, and discharge Wiggly Explorers', its owners, employees, affiliates, and locations from any and all liability, claims, demands, causes of action, or losses for bodily injury, property damage, or other harm that may occur to me, my child(ren), or any other accompanying person(s) arising out of or related to participation in the sensory play class or in group participation.

 

No Childcare Provided: I understand that Wiggly Explorers does not provide childcare services. This is not a drop-off program. I, as the parent / guardian, must remain fully responsible for my child(ren)'s safety, behavior, and well-being at all times.
